Hudson Valley District | November CE Event "The Field of Elder Care Management" Wednesday, November 19, 2025: 7:00 p.m. - 9:00 p.m.
The field of eldercare/geriatric care management affects the physical therapy profession. We will review the history of the field with a focus on the Aging Life Care Association which is a network for healthcare professionals certified in care management for older adults and those living with disabilities. Credentials for membership and the organizations code of ethics will be reviewed. Care management areas of practice which include the following: Advocacy, Assessment, Care Management, Consultation, Counseling, Crisis Intervention, Education, Entitlements, Guardianship, Home Care, Information and Referral, Insurance and Placement
The focus of management will be explained and case examples will be provided.

Course Presenter Miriam Scholl, LMSW Founder of Westchester Elder Care Consultants
Miriam SchollI, LMSW, is the founder of Westchester Elder Care Consultants, LLC, a private Geriatric Care Management practice serving Westchester County. Miriam has been practicing in the field of care management for the past 30 years. She is a New York Licensed Social Worker with a Master's Degree in Social Work from New York University. She brings to her practice extensive geriatric experience having worked in long term care facilities, home care settings, and with corporate eldercare programs. Miriam presented at corporate settings around issues related to the needs of older adults and caregiving issues and presented Guardianship Training programs in Westchester County on Medical and Treatment Issues. She has been an active participant in the Westchester County Senior Law Day Committee and served on the Professional Advisory and Training Committee for Home Health Services of WJCS. In addition, she is a member of the Aging Life Care Association and served on the Board of the New York Chapter for many years. Additional professional affiliations include NASW and Westchester based geriatric committees.
