Finger Lakes District | November CE Event "Empowering PTs Across the Continuum: A Framework for Leveraging Standardized Measures to Drive Interventions and Outcomes" Wednesday, November 12, 2025: 5:30 p.m. - 7:00 p.m.
As physical therapists work across hospital, SNF, IRF, outpatient, and home care settings, they face increasing demands to select and apply outcome measures that reflect patient progress and support clinical decision-making. With diverse patient populations and practice environments, outcome measures play a critical role in linking assessment to intervention planning, documentation, and demonstrating meaningful change.
This session will emphasize strategies for integrating outcome measures into practice, from selection through interpretation, to guide effective intervention design. Using practical case scenarios, documentation examples, and interventions from multiple settings, the session will highlight how outcome measures can be applied to align therapy goals with patient priorities, optimize resources, and improve patient outcomes.

Course Presenter J.J. Mowder-Tinney, PT, PhD Board-Certified Clinical Specialist in Neurologic Physical Therapy, Certified in Neuro-Developmental Treatment & Certified Stroke Rehabilitation Specialist Professor, South College J.J. Mowder-Tinney is a highly experienced physical therapist, holding a PhD and multiple certifications including a Clinical Specialist in Neurologic Physical Therapy (NCS). With over 30 years of expertise, primarily focusing on neuromuscular deficits, she received her education from the University of Colorado – Boulder, University of Miami, and Nova Southeastern University. JJ is recognized for her outstanding contributions, receiving awards such as the Award for Excellence in Neurologic Education from the ANPT and the Marilyn Moffat Distinguished Service Award. She currently serves as a Professor at South College's School of Physical Therapy in Knoxville TN and conducts seminars nationwide on various topics including neurological deficits, spinal cord injuries, and exercise prescription for aging adults. Chris Chimenti, MS, PT Senior Director of Clinical Innovation, HCR Home Care Chris Chimenti earned his Masters of Science in Physical Therapy degree from Slippery Rock University in 1997. He is currently employed as the Senior Director of Clinical Innovation at HCR Home Care, a Rochester-based certified home health care agency operating across Upstate New York State. He has over 24 years of experience in the home health setting and is an accomplished speaker at both regional and national conferences on the topics of home health practice and regulation, clinical research, student affiliation, care redesign, and joint replacement rehabilitation. Chris has conducted a number of studies throughout his career across a variety of special interest areas, including falls prevention, Parkinson’s Disease management, evidence-based standardized measures, pain assessment, sepsis screening, total knee replacement rehabilitation, and home health response to the COVID-19 pandemic. Chris has served APTA Home Health in a variety of leadership roles, including Research Committee Chair, Treasurer, and Vice President. Season Bonino, PT, DPT Board-Certified Clinical Specialist in Geriatric Physical Therapy Clinical Associate Professor, Nazareth University Season Bonino PT, DPT, GCS earned her Bachelor of Science in Clinical Science and her Master of Science in Physical Therapy from Ithaca College, followed by a doctorate in Physical Therapy from Shenandoah University. With more than 28 years of clinical experience across diverse clinical settings including acute and intensive care, skilled nursing, and pediatric early intervention, she integrates this practical expertise into her role as a Clinical Associate Professor at Nazareth University. She is Board certified in Geriatric Physical Therapy by the American Physical Therapy Board of Clinical Specialties. Her clinical and research interests center on balance training and fall prevention for neurologic and geriatric populations as well as on the therapeutic impact of interventions for individuals with Multiple Sclerosis. In addition to her academic and clinical work, she serves on the board of a local non-profit dedicated to improving accessibility in recreational activities and contributes her knowledge as a member of the professional advisory committee for a local home health care agency. Allyson Muehlemann, PT, DPT Board-Certified Clinical Specialist in Neurologic Physical Therapy Adjunct Faculty, South College and Nazareth University Allyson Muehlemann is a dedicated professional with a rich background in physical therapy and education. Having obtained her Doctor of Physical Therapy from Daemen College in 2016 and her Doctor of Education in Education Practice & Leadership from the University of North Dakota in 2023, Allyson possesses a solid academic foundation. She is a Neurologic Certified Clinical Specialist and has completed a Neurologic Physical Therapy Residency at Mayo Clinic, Rochester, MN. Allyson currently serves as adjunct faculty at South College in Knoxville, TN, and Nazareth University, teaching lab- and lecture-based courses in neurologic and cardiopulmonary management, mentoring DPT students in clinical reasoning and evidence-based practice. She actively practices acute care and inpatient rehabilitation physical therapy at Unity Hospital in Rochester, NY, and presents continuing education courses nationally. In addition to her clinical and teaching work, she provides individualized consulting and mentorship for neuro PTs seeking to enhance clinical decision-making, confidence, and impact.
